Everything you wanted to know about adhesion

The durability of a composite restoration depends on the stability of the adhesive bond. The most common problem with composite restorations, whether vital or avital teeth, is the occurrence of microleakage between the margins of the filling and the tooth, which causes marginal caries, staining, hypersensitivity and fracture of the filling. Also, inadequate adhesion procedure (dentin drying, incomplete polymerization, moisture) can cause nanoleakage as the most common cause of postoperative hypersensitivity in patients, but also faster degradation of the adhesive bond. In the case of postendodontic dental restorations, we strive to create the so-called monoblock, an idea based on creating a strong bond between intracanal dentin, posts, and coronary structure.

The lecture will explain the latest adhesion systems and compare them with previous adhesion systems. Their advantages, limitations, clinical work, possibilities of application in restorative dentistry and postendodontic dental restorations will be explained. Also, the latest results of the durability of such adhesion systems and the reasons for adhesion failure will be presented. A number of clinical cases and how to avoid the most common mistakes in clinical work will be presented. Based on current scientific knowledge, recommendations will be made for clinical work and the choice of adhesion system depending on the clinical situation.
