Clinical Case: First molar failing restoration

Using 3M™ Filtek™ Supreme XTE Universal Restorative and 3M™ Scotchbond™ Universal Plus Adhesive.

About the Case

The patient presented for an exam after 6 years with no chief complaint other than making sure he was in good oral health. Clinical examination revealed a cavitated carious lesion in relation to a failing occluso-lingual composite in tooth 26.

The treatment plan consisted of replacing the failing composite restoration using the 3M™ Scotchbond™ Universal Plus Adhesive and 3M™ Filtek™ Supreme XTE Universal Restorative, Shade A1B. After total isolation with rubber dam, the caries were removed and the procedures related to the adhesive were completed. The composite material was placed and polished using the 3M™ Sof-Lex™ Polishing System.

Initial situation

Initial situation shows cavitated carious lesion in the existing OL composite on tooth 26. After anesthesia, rubber dam was placed for ideal isolation.

Preparation

Caries were fully excavated and the rest of the failing composite removed.

37% phosphoric acid etch gel was applied using the selective etch technique. This was left on for 15 seconds, then rinsed away with water.

3M™ Ketac™ Universal Aplicap™ Glass Ionomer Restorative

After rinsing and drying, an evident etch pattern was showing on the enamel only.

Application of adhesive

3M™ Scotchbond™ Universal Plus Adhesive was applied with agitation for 20 seconds and air-dried for at least 5 seconds until the adhesive no longer moved. Active application is the key for best results.

Preparation ready for bonding

The universal adhesive was then light cured for 10 seconds.

3M™ Filtek™ Supreme XTE Universal Restorative A1B was placed in multiple layers to obtain a well adapted and esthetically pleasing restoration.

3M™ Sof-Lex™ Pre-Polishing Spirals (Beige)

Once the finishing and polishing were completed, the final result shows excellent color integration, and anatomy that corresponds to the tooth.
